One of the things that would be considered a core conclusion from across the readings and class session pertinent to service delivery for older adults is providing for hospice care and health needs of the older adult. Apparently the process is ever changing, complex and very dynamic (Jennings & Murray, 2005). The older adult population is on the increase and the challenges multiplying as well for many people. The needs at adult age form a wider scope and therefore call for a better comprehension for the concerned to act swiftly and appropriately. There is need for research on better innovative methods of accommodating such needs. The older adults are faced with many complex health and old age issues. Many of them live with acute conditions which could be more than one affecting them daily (Johnson, 2005).

Significance to the Federal Government

Policymakers at the federal level should be aware of this problem that affects older adults and therefore must make every effort in addressing their problems. The older adults need a lot of care because they cannot just be left out to die unattended to. The burden of their care is often left in the hands of the immediate family; members who form part of the federal government’s labor force.  With the increasing number of older adults, it is obvious that the dependency ratio in any country will certainly be high. The older adults are adding to the number of dependants amongst the young in the society (Jennings & Murray, 2005). With this increase in the number of dependants, the federal government will directly be affected by the shifting the labor to cater for the older adults. Low productivity owed to the attention given to the older adults is expected to cost the government more than if the government could assist in providing for the hospice care and health needs of the aging in the society. This is a very sensitive matter to be left in the hands of the immediate family alone. It requires concerted efforts both from the family and the federal level policy makers (U.S. Department of Health & Human Services, 2000).

While death cannot be avoided, there is no guarantee for dying in a bad manner (Johnson, 2005). Care for the older adults must be provided until their last breath. It is inhuman and against human rights to let the older adults die from their chronic conditions which would otherwise have been taken care of. It is therefore constitutional leave alone being an economic measure to be taken for the welfare of the nation. Therefore, the provision of hospice care and health needs is very paramount in the end of life of the older adults (Berenson & Howell, 2009). It is expected that the government sees the need from this perspective as well as the sense that it is protecting the rights of the older adults and the vulnerable to chronic ailments.  Policy makers at the federal level are hence expected to make such policies that create such awareness promoting the welfare of the older adults (U.S. Department of Health & Human Services, 2000).

Having a good understanding of the issues experienced by the older adults is of great importance in dealing with the ravages of chronic conditions of older adults. This will help in establishing proactive measures geared towards the welfare of all in the society. It is very important that a good policy is laid down on measures to be applied in addressing the problems of older adults. It is thus a national issue and not a problem of those linked to it at a given time. The same way services are provided to the young ones, so it is expected for the aging. It does not necessary have to be absolutely free. A subsidization made to that effect would count a lot in mitigating the difficulties realized by caregivers and/or families.

Program

Berenson and Howell (2009) describe a program aimed at structuring, financing and paying for effective chronic care Coordination of the older adults. This topic incorporates pertinent outcomes of the coordination of care for the older adults. The coordination of care is described as responsive, proactive, consensual and continuous coordination of mental health, services of social support and medical care for an older individual and the caregivers/family. This program is an interdisciplinary methodology based on careful evaluation procedures applied to integrate social support and health care services where the coordinator of care comes up with good structures of managing finances and monitor the needs of the individual and the preferences thereof founded on a comprehensive strategy.

The strategy of search for the topic was based on reports got from medical centers and health institutions, social work abstracts and sociological collections as well as primary research carried out by individuals on the same subject.  All these sources were tailored to the requirements of the topic in question. The synonyms and concepts studied were organizational loci for effective chronic care coordination as well as financing and payment options in: care coordination; medical principal/primary care practices; integrated systems of delivery; hospitals; community organizations; integrated care programs and medical advantage plans and the implications on policy. Any of the above areas of research were limited by older adults, journal articles, and English publications and published between 2003 and July 2009.

Before implementing the program, there is much communication needed that highlights the societal and national challenges faced whilst taking care of the older adults. It should be realized that the older adults are our own people and need to be taken care of. The care given to the older adults would require resources. These resources will mean that some cost is to be incurred by the affected. The affected people in this case will also transfer the problem to the federal government. The federal government relies on the tax payer and others for her revenue to function. Therefore, there needs to be such a kind of understanding that implementing such a program/policy would be for the welfare of the entire nation. All stakeholders must come to an understanding of this so that the program can be implemented effectively.
